Pre-Registration Options |

We
ask that you pre-register once your Physician has scheduled your
surgery, procedure or test. You may only pre-register through our
online application at least three (3) business days prior to your
service. To pre-register within three (3) business days of your
scheduled service, please call our Admissions Department at
281-633-4074. |

Patients
who complete the on-line pre-registration still need to stop by the
Admissions Dept to complete and sign Consent Forms. The Admissions Dept
is located at OakBend’s Main Hospital Entrance. For any questions,

In
order to make sure your registration process goes smoothly, please have
the following information ready when you arrive: • Current insurance
card • Picture ID • Any orders/prescriptions the Physician may have
given you. |

Upon
your arrival, you will be asked to: - Sign in at the front desk - The
Registration representative will ask you to verify your current
information and sign consent forms, etc. - You may be required to pay a
co-payment during this time. The amount of co-pay varies and is based
on the amount your insurance will cover. |
